Workshop/individual Class Sign-up Form

Once you fill out this form you will be contacted to arrange an evaluation of your dog so that she/he is put in the appropriate class. This may be done in person or over the phone depending on the class you sign up for. Your registration will need to be accompanied by vaccination certificates. You may scan them and email the PDF format file to gooddog@skypilotdogtraining.com, or send photocopies in the mail with your cheque.
